If you elect to freeze any embryos, and/or you require intracytoplasmic sperm injection (ICSI) or assisted hatching, there is no additional charge.
These fees DO NOT include MEDICATIONS or:
A new patient visit |
The cost of mandatory infectious disease screening and other required testing of the patient and her partner done prior to treatment |
Laboratory tests ordered during treatment to monitor a patient’s response to medication |
Follow up pregnancy tests, office visits, ultrasound examinations, or laboratory testing to monitor a resulting pregnancy, or procedures related to pregnancy. |
Regardless of any insurance coverage, fees must be paid in full prior to beginning medication for treatment , at the time of your initial IVF interview. There are no payment plans available or refunds if the procedure does not result in pregnancy or a live birth. If a repeat procedure is necessary, you MAY be eligible for a discount for a repeat cycle. Gulu Independent Hospital. accepts cash, local checks, and credit cards. If any of the services in the fee schedule above are not rendered because of cancellation of your cycle, you will receive a refund for the difference. Gulu Independent Hospital will make every effort to help you bill and collect from your insurance company by preparing your claim form; however, we cannot assure you of insurance reimbursement. We recommend that you bring us a completed claim form from your insurance carrier.
